When People Are Deaf to Music

A small but significant proportion of the population are unable to perceive music as most people do -- and their condition is revealing new truths about how the brain processes sound

A Music is so deeply embedded in human experience that it is easy to assume everyone perceives it in roughly the same way. For the vast majority of people, music produces immediate and powerful emotional responses -- pleasure, sadness, excitement, calm -- and these responses appear to arise automatically, without any need for training or conscious effort. Yet a small but consistent proportion of the population, estimated at somewhere between four and five percent, report that music leaves them entirely unmoved. They can hear it -- it is not a hearing impairment -- but they derive no pleasure or emotional significance from it. This condition, now known as musical anhedonia, has only recently been formally recognised as a distinct psychological phenomenon.

B Musical anhedonia should be distinguished from amusia, a related but separate condition. Amusia is a disorder of music perception in which affected individuals are unable to detect changes in pitch, recognise familiar melodies, or identify when a note is out of tune. People with amusia often describe music as an unpleasant noise rather than an organised sound. The condition is present from birth in some individuals and can also arise following brain injury. Musical anhedonics, by contrast, have entirely normal music perception -- they can detect pitch changes, recognise melodies, and judge whether notes are in tune -- but the sound simply does not engage their emotional response systems in the way it does for most people.

C Research into musical anhedonia has converged on the connection between the auditory cortex -- the brain region that processes sound -- and the reward system, particularly the nucleus accumbens, which registers pleasure and motivation. In people who experience strong emotional responses to music, neuroimaging studies consistently show robust communication between these two regions: as a piece of music builds toward an emotionally charged moment, activity in the reward system increases in anticipation, and peaks at the moment of maximum emotional intensity. In musical anhedonics, this functional connection appears to be weaker, meaning that even though the music is accurately perceived, the signal fails to reach the reward system with sufficient strength to generate a subjective sense of pleasure.

D The discovery of musical anhedonia has had practical implications for music research more broadly. Earlier studies of the neural basis of musical pleasure typically recruited participants from the general population without screening for their relationship to music, a practice that would have systematically included musical anhedonics in the sample. Because these individuals respond to music differently at the neural level, their inclusion would have diluted the signal seen in group averages, potentially causing researchers to underestimate the strength of the brain's response to music. The development of validated questionnaire measures for musical reward sensitivity has allowed researchers to screen participants more effectively and to study the range of individual differences in musical response in its own right.

E Musical anhedonia raises interesting questions about whether the emotional power of music is universal or culturally acquired. If music's effects on the brain were entirely the product of cultural exposure and learned associations, one would expect musical anhedonia to be evenly distributed across populations -- a random variation in individual history. But if the condition reflects a genuine difference in the structural connectivity of the auditory-reward pathway, it suggests that the capacity to be moved by music has a biological component that varies between individuals regardless of their cultural background. Evidence tends to support the latter interpretation, with neuroimaging and genetic studies both indicating that the strength of auditory-reward connectivity is partly heritable.

F The existence of musical anhedonia does not imply any broader deficit in emotional life or social functioning. Individuals with the condition experience pleasure, motivation, and emotional engagement in response to other stimuli -- food, social interaction, physical activity -- in entirely normal ways. Their brains' reward systems are fully functional; they simply do not respond to music as a source of reward. In this respect, musical anhedonia resembles other specific sensory preferences: just as some people are indifferent to the visual arts or to certain flavours of food, some people appear constitutionally indifferent to music, not because something is wrong with them, but because of normal variation in how the brain weights different types of sensory experience as rewarding.
